DIRECTIONS FOR SAVING DOCUMENTS:
Open any file below by clicking on "download file". BEFORE you enter any information go to the upper left hand corner of your screen and click on "file", then go to "save as". Save the document wherever you would like to (i.e. in the H drive under a file you create called "Evaluations").
